Permissions to go head with works can be either verbal or signatures received via E-mail.
There are no hidden fee's. You will be notified of a "Poor quality dictation" immediately on receipt of such audio. You will be issued with a corrected quote and turnaround time. I endeavour to produce an accurate document but in the event of very poor quality transcription I will either contact you immediately or such problem areas in the text will be audio-marked as [unclear 00:00]. Please note it will take a transcriptionist longer to produce such a document.
The final proof read is the sole responsibility of the client. Please notify me within 48 hours, should you wish to make any alterations/additions (within reason) to the document I will charge per audio minute.
All incoming emails, discs, CD's or other media are scanned for viruses. I will not open any unsolicited attachments to emails. All emails and attachments returned to clients are scanned for viruses - however it remains the responsibility of the client to check text, and any such attachments for viruses/worms/trojans as we do not accept any responsibility.
Paragon reserves the right to refuse any material content deemed inappropriate and will terminate works immediately.
CONFIDENTIALITY.
In accordance to Caldicott principals, patient identifiable information will where possible will not be used unless previously discussed with Clinician/Clinical Specialist.
All clinical digital audio dictations will be transferred via the secure server unless otherwise agreed.
Any analogue cassette will be either collected in person or transported via special delivery.
The secure server operates a personal client log in and password. This will be discussed and arranged on commencement of any works. You must not disclose your personal log in and password.
If you wish, we can agree and produce a confidentiality agreement prior to the commencement of any works. Although I have signed, dated confidentiality agreements between all our transcribers and we can produce a copy of these for you on request.
